I hereby apply for the waiver of above service fee. I shall understand and agree below terms:

I shall not participate in the monthly maintenance service scheme and pay certain monthly maintenance service fee. In any circumstances, if I ask for the checking or maintenance service for LP gas appliances provided by IP&E, checking or maintenance service fee will be charged from time to time. Currently the mentioned service fee is HK$_350_ (not include accessories or other repair items). Please note that the prior notification of price change will not be provided.

If I would like to participate in the service scheme in the future, the mandatory checking of LP gas appliances with service charges will be performed that I shall accept.

I understand IP&E has the right to request me to provide the LP gas appliances information and past checking/maintenance record for service purpose.
